Provide your full legal name, Date of Birth, Phone Number, last 4 digits of your SSN (or Tribal Id), Address Etc. (The information is collected as per FCC guidelines).
Provide your consent to receive the Lifeline benefit through Integrated Path Communications, LLC (IPC). Consent Form is not needed for Retail Customers.
National Verifier is Government's centralized system, it determines the Lifeline eligibility based on the information provided in Step1. Not needed for Retail Customers.
Lifeline customers can simply proceed to complete the enrollment with a Free SIM. Retail customers will be required to make a payment to complete the enrollment.

Integrated Path Communications, LLC (IPC) is a service provider for the government-funded Lifeline Program. Lifeline is a government benefit program operated by the Federal Communication Commission (FCC) that provides subscribers a discount on qualifying monthly telephone service, broadband Internet service, or bundled voice-broadband packages purchased from participating wireline or wireless providers. Lifeline provides up to a $9.25 monthly discount on service for eligible low-income subscribers and up to $34.25 per month for eligible subscribers on Tribal lands. Subscribers may receive a Lifeline discount on either a wireline or a wireless service, but they may not receive a discount on both services at the same time. Lifeline also supports broadband Internet service and broadband-voice bundles. FCC rules prohibit more than one Lifeline service per household. The Lifeline related Plans are available for QUALIFIED CUSTOMERS only. When the Lifeline ends or a customer/s household is no longer eligible, the customer/s will be subject to our regular undiscounted rates.
